HR Custom Forms and Picklist Values

System Administrators can configure record intake in their application by creating a custom record type and updating the layout of a standard record type (e.g. case, party, and note). You can create or edit any form layout in the Case IQ Form Builder. The following articles cover how to use the Form Builder for custom and standard form layouts:

HR Forms and Picklist Values

Instead of setting up custom forms yourself, you can download a configuration file from the table below and import it into your Case IQ application. These files contain (1) case, party, and profile fields, (2) picklist values, and (3) custom forms configurations. This article also describes the custom forms in the configuration file and details additional custom forms you may want to build in your application. Refer to the Configuration Export and Import article to learn how to import this configuration into your application.

The configuration file includes the following form types:

  1. Issue: records all HR-related incidents associated with a case, such as Harassment, Discrimination, Conflict of Interest, Leave of Absence, Workplace Accident, Policy Violation, etc.
  2. Outcome: stores information on the outcome (substantiated, unsubstantiated, inquiry only, etc.), and the corrective Actions (coaching, warning, resignation, suspension, termination, etc.) for each "Issue" record of a case.
  3. Recovery: records the recovery of anything related to a case by type, such as Material/Product, Monetary, or Property. 
  4. Interview: captures data on interviews conducted with involved parties, such as a complainant, subject, or witness, to gather the details of the incident.

Records any evidence, proof, or corroborating documentation collected for a case.

Detailed Interview Form

Walks an interviewer through interview best practices to capture specific misconduct violation information from an interviewee. This complex form is divided into 3 tabs to separate the questions intended to be asked in the interview and questions for the interviewer to describe the interview event and assess the interviewee's credibility. By adding the visibility rules detailed for each section, you can create a responsive form that will load sections on the interview tab based on the "Interview Type" field selection.
