Complete a Request Form

Investigators may request your input when processing a case to capture information about the incident, seek approval on the investigation proceedings, or gather feedback on the outcome. To fulfil this request, you can fill in a form in the organization's case management system.

You will receive the following emails if a caseworker has requested your assistance with a form:

  1. A message from someone on the investigation team, with a secure link to the form.Email with a link to the form someone has requested you to fill in.
  2. The “Verification Code” email, which contains your unique login credentials.The verification code email, with which you can access the form.

You can access the form using the following steps. We are using an example Witness Statement form to demonstrate.

  1. Click the "Access the Form here" link in the first email to load the form in your web browser. You will see the link here:Link to the form in the first email.
  2. Your browser will load the page to access the form. In your web browser, click the checkbox in Step 1 to acknowledge that a read receipt will be sent to the organization and your responses will be securely stored.
  3. Go back to your email and copy the verification code in the second email. Make sure to copy only the letters and numbers, not the spaces before or after the password. You will find your verification code here:Verification code in the second email.
  4. In your web browser, paste the verification code into the “Enter verification code” box in Step 2.
  5. Click the Access Form button.
  6. You will now see the form you have been requested to complete. Enter your responses in the form's fields. You will not be able to edit your responses after submitting the form.
  7. When you are finished filling in the form, click the Submit button to send your responses to the investigative team.

Opt Out of Responding

If you do not want to complete the form, you may be able to decline the request. If you can opt out of responding, you will see a Decline button at the bottom of the form. If you click this button, the investigation team will be notified that you declined to complete the form.
